What to do if you have lost your card

If you have lost your card and wish to receive a replacement you should write to us at the address below.  You will need to include two photographs and payment of £10 to cover administration costs. Your new card will have the same expiry date as your previous card.  Please enclose details of your name and NI number and send to:
